We have built a family practice around long term health and maintenance of your ideal smile. Please share with us as much information as you can including how you feel about your teeth and what your dental goals may be. Help us achieve the smile of your dreams.
We accept a variety of insurance plans and will be happy to help you maximize and manage your benefits. Whenever possible we file claims on your behalf.
Some insurers may require pre-authorization prior to treatment and we will verify that these are approved before proceeding, or alternate financial arrangements are agreed upon.
Co-payments are due at the time of service and you will be responsible for any deductibles and unpaid balances.
We take cash, checks, credit cards and Care Credit.
We understand that healthcare costs can place a financial burden upon our patients. If you are concerned about financing treatment we will assist you in establishing a manageable payment arrangement.
Cancellation and No-Show Policy
Your appointment time is reserved exclusively for you. This allows us to serve all of our patients as quickly as possible and prevent you from having to wait for care.
We reserve the right to charge for any appointment(s) broken without 48 hours’ notice. The charge will be $75 per half-hour of hygiene scheduled and $150 per half hour scheduled with the Doctor. Fees must be paid in full prior to the patient’s next appointment.
Habitual missed/canceled/rescheduled appointments may result in a patient being required either to pay up front prior to scheduling an appointment or this office may longer be available to provide your dental services. Thank you for your understanding.
